Proof of Residency Requirements - Required for all new players. If you have moved outside Tracy's boundaries and played for Tracy Little League last year contact Chris Hewitt for proper waiver.

Medical Release Form - Will need to be completed and turned into league prior to tryouts or playing.

Preliminary Accident Report Form - To be completed by team manager and submitted to safety officer with 48 hours of incident.

Volunteer Application - ANYONE who has repeated consistent contact with players must have this form completed prior to participating in any way. If you would like to help at practice, even ONCE, this form needs to be completed. Please contact Chris Hewitt is you have any questions.
